Office Policies

Thank you for visiting our web site. Below is some information that hopefully will answer many of your questions about how the office handles various issues.


After Hours or Emergency Care
There is a physician on call each night and on the weekends.  If you have an urgent medical need, please call the office number then press zero and the attendant will get in touch with the physician on call.  If you are experiencing an emergency, dial 911.
 
Appointments:
We see patients by appointment only. We do not take walk-ins, but we do try to make same day appointments for unexpected acute illnesses. To schedule appointments, please call our office.  We request that you inform our staff of the nature of your visit so the appropriate amount of time can be allotted.  Patients with previously scheduled appointments will be given priority.  If you are more then 15 minutes late for your appointment, we reserve the right to reschedule the appointment. Please arrive early for your first appointment in order to complete the appropriate forms.

Cancellations:
Please call our office at least 24 hours in advance to cancel an appointment.  Our office staff will be happy to reschedule your appointment.  Should you miss your appointment and fail to notify the office there will be an additional charge. If a patient frequently miss appointments, that patient may be discharged from the practice at the discretion of East Houston Medical Group.

Medical Information:
If you are a new patient, we would appreciate you bringing the names of your previous physicians. At each appointment, please bring all medication bottles, inhalers, and anything else pertaining to your daily healthcare routine.

Payment Policy:
Payments and co-pays for all services are due at the time of each visit.  We gladly accept cash, checks, MasterCard, and Visa.  Our clinic will make payment arrangements when necessary.

Insurance:
We do accept a variety of insurance plans.  Patients are to present their insurance card at each visit.  We encourage the patients to fax or drop off their insurance card before their scheduled appointment.  Deductibles and co-pays are collected at the time of service.  Please remember that your insurance contract is between your carrier and you. You will be responsible for any amount not covered by your insurance company.
For all third party claims, the patient is responsible for all office visit charges.  The patient can then be reimbursed by the insurance company or their attorney.

Prescription Refills:
Please call your pharmacy for prescription refills.  Please allow two business days to complete the request.  Prescriptions and refills will not be completed after hours or on weekends. We do not call in antibiotics or pain medication on weekends or after hours.

Lab and Diagnostic Results:
It will take approximately 7 to 10 working days for results to be received by the clinic.  You will be informed by a letter or phone call with the lab results.

Referrals and Authorizations:
Referral and authorization requests are handled here in the office. Please allow at least 5-7 working days for non-urgent requests. You will be notified when the request has been approved. If you have not been contacted after 7 business days, please call us back. Referrals will not be handled after-hours or on weekends.


Medical Records
All information in the medical record is confidential and will not be released without your consent.

Other items
The completion of additional forms and paperwork that is requested outside of an office visit may have additonal costs. (i.e. FMLA papers, physical exam forms, additional vaccine record forms)
